Like...It is diametrically opposite of how it should be set up. Senior leadership screw-ups should garner MORE swift and brutal reprisal than lower-end. That's a fundamental part of how good hierarchies work.

General MacArthur wasn't charged and court-martialed for openly rebelling against Presidential authority and communicating with foreign diplomats, including enemy ones, behind the President's back to force America into a nuclear war because he was so popular that he was basically untouchable. MacArthur was only removed from command positions that he held at the President's discretion, and that was still something the President took substantial political damage from doing.
