Presumably he has a mild blunder or accidentally loses and sorta embarasses himself mid fight

So not even Aerys could come up with a reason to get Jaime as a hostage/bodyguard

What happens with Jaime stuck as Tywin’s heir? Who does he marry? How well does he do?

Well, Joanna Lannister's plan with the Princess of Dorne and Rhaella Targaryen had been for Jaime to marry Elia and Rhaegar to marry Cersei.

There's actually been enough things that happen at Harrenhal that I may forgotten which one that was. You are right. The triple marriage plan is dead and buried at that point.

OTL he gave the order when Lannister army turned cloak, the combined forces under Ned's command were still some distance from the city, though probably close enough to see the fireworks.

However there might one earlier butterfly, Jaime becoming the Kingsguard was the last straw for Tywin, if he stayed as King's Hand he might have crushed the Robert's rebellion, unless the murder of Starks proved to be the last straw.

Tywin still would have had issues with Aerys’ thinking he was a threat, though. That is why Aerys took Tywin’s heir.
