Given Troi's entire purpose was to be the ship's counselor and psychologist, and she called Data out on the silliness of having a Sigmund Freud holodeck session instead of going to her as the ship's professional, I don't think we can fairly say there weren't therapists available.
Troi couldn't muck about in anybody's head. It was literally a plot point several times. She could sense emotions but couldn't sense actual thoughts and definitely couldn't send anything, much less muck about in heads.Forgive me if I'm not confident with having a telepath mucking about in my head.
