He has his canon intelligence but with near-instinctual attraction or desire to smith/tinker/engineer

His body being more stout yet muscled means that Tywin might decide to let him get squired if only to avoid seeing him

How do events play out? Presume Joanna still dies

Bonus: Tyrion can carve magic runes somehow

Tywin and Tyrion's relationship wouldn't be quite as cold, for one. Whilst Joanna's death is a very real wedge between them, Tywin would note that his son is "different" instead of "deformed." Everything is in working order, he is actually quite stout and muscled thus martially capable, very creative and wickedly intelligent.

He may well tolerate him as heir to Casterly Rock. Not his ideal choice but he can stomach it this time.

I don’t think Tywin would allow Jaime to simply stay on the Kingsguard still, though

Not if Jaime's got any say in it. He remained quite adamant on remaining in the Guard and only, regretfully, yielded to his father in order to save Tyrion's life. Not to mention, with Tyrion being in an entirely useful, strong state, I doubt Tywin would be as eager to get rid of him/let Cersei rig the trial as badly she did. Indeed, he'd be breathing down Tyrion's neck to get a proper wife and start pumping out more Lannisters to secure his legacy.

So I think the big change in terms of father-son relationship here, is Tywin would be more apathetic to Tyrion instead of hating him.

Though, canon Tyrion with the body of a Fantasy Dwarf still feels kinda awesome, if only to see him smash Gregor Clegane to bits on his own
